The MajesTEC-5 ( Abstract #493 ) and MajesTEC-4 ( Abstract #494 ) studies establish the potential of TECVAYLI ® for use in newly diagnosed patients, with promising efficacy and a tolerable safety profile. In the clinical trial, CRS occurred in 72% of patients who received TECVAYLI ® at the recommended dose, with Grade 1 CRS occurring in 50% of patients, Grade 2 in 21%, and Grade 3 in 0.6%. Recurrent CRS occurred in 33% of patients. Most patients experienced CRS following step-up dose 1 (42%), step-up dose 2 (35%), or the initial treatment dose (24%). Less than 3% of patients developed first occurrence of CRS following subsequent doses of TECVAYLI ® . The median time to onset of CRS was 2 (range: 1 to 6) days after the most recent dose with a median duration of 2 (range: 1 to 9) days. The most frequent neurologic toxicities were headache (25%), motor dysfunction (16%), sensory neuropathy (15%), and encephalopathy (13%). With longer follow-up, Grade 4 seizure and fatal Guillain-Barré syndrome (one patient each) occurred in patients who received TECVAYLI ® . In the clinical trial, ICANS was reported in 6% of patients who received TECVAYLI ® at the recommended dose. Recurrent ICANS occurred in 1.8% of patients. Most patients experienced ICANS following step-up dose 1 (1.2%), step-up dose 2 (0.6%), or the initial treatment dose (1.8%). Less than 3% of patients developed first occurrence of ICANS following subsequent doses of TECVAYLI ® . The median time to onset of ICANS was 4 (range: 2 to 8) days after the most recent dose with a median duration of 3 (range: 1 to 20) days. The most frequent clinical manifestations of ICANS reported were confusional state and dysgraphia. The onset of ICANS can be concurrent with CRS, following resolution of CRS, or in the absence of CRS. Monitor patients for signs and symptoms of neurologic toxicity during treatment. At the first sign of neurologic toxicity, including ICANS, immediately evaluate patient and provide supportive therapy based on severity. Withhold or permanently discontinue TECVAYLI ® based on severity per recommendations and consider further management per current practice guidelines. Due to the potential for neurologic toxicity, patients are at risk of depressed level of consciousness. Advise patients to refrain from driving or operating heavy or potentially dangerous machinery during and for 48 hours after completion of TECVAYLI ® step-up dosing schedule and in the event of new onset of any neurologic toxicity symptoms until neurologic toxicity resolves. Hepatotoxicity - TECVAYLI ® can cause hepatotoxicity, including fatalities. Elevated aspartate aminotransferase (AST) occurred in 34% of patients, with Grade 3 or 4 elevations in 1.2%. Elevated alanine aminotransferase (ALT) occurred in 28% of patients, with Grade 3 or 4 elevations in 1.8%. Elevated total bilirubin occurred in 6% of patients with Grade 3 or 4 elevations in 0.6%. Liver enzyme elevation can occur with or without concurrent CRS. Monitor liver enzymes and bilirubin at baseline and during treatment as clinically indicated. Withhold TECVAYLI ® or consider permanent discontinuation of TECVAYLI ® based on severity. Infections - TECVAYLI ® can cause severe, life-threatening, or fatal infections. In patients who received TECVAYLI ® at the recommended dose in the clinical trial, serious infections, including opportunistic infections, occurred in 30% of patients, with Grade 3 or 4 infections in 35%, and fatal infections in 4.2%. Monitor patients for signs and symptoms of infection prior to and during treatment with TECVAYLI ® and treat appropriately. Administer prophylactic antimicrobials according to guidelines. Withhold TECVAYLI ® or consider permanent discontinuation of TECVAYLI ® based on severity. Monitor immunoglobulin levels during treatment with TECVAYLI ® and treat according to guidelines, including infection precautions and antibiotic or antiviral prophylaxis. Neutropenia - TECVAYLI ® can cause neutropenia and febrile neutropenia. In patients who received TECVAYLI ® at the recommended dose in the clinical trial, decreased neutrophils occurred in 84% of patients, with Grade 3 or 4 decreased neutrophils in 56%. Febrile neutropenia occurred in 3% of patients. Monitor complete blood cell counts at baseline and periodically during treatment and provide supportive care per local institutional guidelines. Monitor patients with neutropenia for signs of infection. Withhold TECVAYLI ® based on severity. Hypersensitivity and Other Administration Reactions - TECVAYLI ® can cause both systemic administration-related and local injection-site reactions. Systemic Reactions - In patients who received TECVAYLI ® at the recommended dose in the clinical trial, 1.2% of patients experienced systemic-administration reactions, which included Grade 1 recurrent pyrexia and Grade 1 swollen tongue. Local Reactions - In patients who received TECVAYLI ® at the recommended dose in the clinical trial, injection-site reactions occurred in 35% of patients, with Grade 1 injection-site reactions in 30% and Grade 2 in 4.8%. Systemic administration-related reactions occurred in 7% of patients with the first injection, 0.2% with the second injection, and cumulatively 1% with subsequent injections. The median time to onset was 2.9 hours (range: 5 minutes to 3.5 days). Of the 165 systemic administration-related reactions that occurred in 93 patients, 144 (87%) occurred on the day of DARZALEX FASPRO ® administration. Embryo-Fetal Toxicity Based on the mechanism of action, DARZALEX FASPRO ® can cause fetal harm when administered to a pregnant woman. DARZALEX FASPRO ® may cause depletion of fetal immune cells and decreased bone density. Advise pregnant women of the potential risk to a fetus. Advise females with reproductive potential to use effective contraception during treatment with DARZALEX FASPRO ® and for 3 months after the last dose. The latest salvo in an ongoing feud between Calumet City Mayor Thaddeus Jones and Clerk Nyota Figgs was delivered Wednesday night as a lawsuit involving both parties was dismissed by members of the City Council during a special meeting held outdoors on the steps of City Hall. Ald. James Patton, 6th Ward, said he called the special meeting after repeated failures to place a motion to dismiss the lawsuit on regular city meeting agendas. He said the mayor took control of the agenda from the clerk and is preventing council action on the lawsuit, which claims Figgs unlawfully destroyed city records. “I think the lawsuit is nonsense,” Patton, who is running against Jones for mayor in 2025, said Thursday. He added he believes the allegations were based on false information collected by city auditor Alyssia Benford. Benford recently challenged Figgs’ reelection as city clerk based on her city of residency, while Figgs is objecting to Jones’ candidacy based on his residency and that he holds two government offices. Both challenges are up for consideration Friday by the city’s electoral board. Jones sent a notice to aldermen Wednesday afternoon canceling the special meeting and all other business scheduled that day at City Hall “due to an emergency pest control and water piping issues” within the building, according to the obtained email. Patton said he expected Jones to interfere with the meeting due to his personal interest in the lawsuit, and reported staff changing locks to City Hall entrances and barricading the building shortly before the scheduled meeting. He said he notified the mayor’s office in advance that the council would hold the meeting outdoors without the mayor presiding if he refused to cooperate. Jones said in a statement that aldermen could have rescheduled the special meeting and that the council acted without proper authority. He cited an ordinance that only the mayor can direct the city’s litigation. “This is a political ploy by a failed candidate and a city clerk who destroyed city records,” Jones said. An attorney representing the city in the lawsuit, Keri-Lyn Krafthefer, of the firm Ancel Glink, said Thursday she was awaiting direction from the mayor on whether to drop the lawsuit. She also said Jones is the only person with authority on Calumet City legal matters. The case stalled Monday after Judge Alison Conlon recused herself after disclosing she may have distributed literature for Jones when he was running as a state representative in 2016. Patton said if Jones’ attorneys move forward with the lawsuit he will call for another special meeting to motion to dismiss the attorneys. The lawsuit’s dismissal was approved during the special meeting from all those who attended, including Alds. Nine business days into her tenure as the executive director of Fish and Game, Stephanie Simek was called Thursday to present the department’s vision for the next state budget. Complicating her early-tenure task: At a time when agencies are preparing for a tighter budget ahead, Fish and Game faces significant, long-standing financial challenges, those within the department have said and a bipartisan legislative study committee concluded in a November report. “Testimony given throughout our study committee meetings indicated the Fish and Game Department is headed to a financial shortfall,” the report warned. “It needs either to increase General Fund funding, increase fees, find areas to cut spending, or look hard at reducing some of the services it offers.” The report pointed to the fact that “Fish and Game is continually tasked with many additional and time-consuming activities beyond just game management,” and that it faces “a multitude of funding challenges across its divisions,” including “many responsibilities and expenses that don’t directly affect sportsmen and wildlife.” Fish and Game has a wide range of tasks, such as managing hunting and fishing licensing and rules; executing search-and-rescue missions for hikers, children, Alzheimer’s patients, and those in mental health crises; helping homeowners with wildlife issues; and maintaining programs related to nongame and endangered species, among many others, the report said. It gets only a tiny sliver of its budget from the general fund, while about a quarter each comes from license fees and federal funds, and nearly a quarter comes from off-highway recreational vehicle funds. New Hampshire, like the rest of the globe, also faces mounting environmental problems that complicate the agency’s work. Simek pointed to stressors like “climate change, declining species populations, habitat changes,” and diseases. About 70 percent of “dedicated revenue for fish and wildlife agencies in the Northeastern United States is generated through license sales and federal grants,” Simek said. “The remaining 30 percent is typically through general fund or special sources, such as permanent fees or specialty stamps or lotteries, depending on the state.” In New Hampshire, she said, “my understanding is ... about 3.5 percent of our budget is general fund.” Simek noted in her presentation, which came near the end of three-day budget hearings at the Legislative Office Building in Concord, that Fish and Game’s budget proposal was crafted before her arrival at the department, but she was clear about the challenges the department faced. A lack of sustainable funding has been cast as a serious threat to the department’s survival by those close to it. “I don’t know how much longer Fish and Game can go on unless we get sustainable funding,” Ray Green, chair of the Fish and Game Commission, told the study committee this summer. He said the commission felt the department needed to get more of its budget — perhaps 15 to 20 percent — from the general fund to achieve that sustainability. The study committee report, citing its limited time and resources, said it could not recommend a specific sum of money that the department may need from the general fund. Simek warned the budget panel about the impact cuts could have on the agency and the wildlife and public it is tasked with serving. By the time she made this case for support, Gov.-elect Kelly Ayotte was back in the hearing room. Ayotte will present a budget proposal early next year soon after she takes office. “I am realistic that we are already operating on a very skeleton budget,” Simek said. “We’ve met the 4 percent decrease, but I’m not sure that we’re likely to fill the entire gap without legislative support. If there are financial cuts to our funding, we will need to likely cut services, and as a new director for an agency that’s mandated to serve the public and our natural resources, reducing services is very concerning to me and will result in collateral and direct negative impacts on our natural resources, our public, and our economy. I really don’t want to see that happening, so I’m hoping we’ll have your support.” Kathy LaBonte, the department’s business division chief, said the department had 192 full-time employees. It has five unfunded positions, which remain unfunded in the new budget, she said. “We did meet the governor’s targets for use of Fish and Game funds,” she said. “However, in order to do that and maintain all programs and services, we used up the entire balance in the Fish and Game Fund at the end of the biennium, leaving only approximately $100,000 in there.” A sheet prepared by the department showed a growing projected gap in the coming years between the department’s unrestricted revenue and Fish and Game Fund expenses. By 2027, the department is budgeted to have nearly $5 million more in expenses than it does in revenue, the sheet projected. It is not the first time Fish and Game has stared down this reality. Scott Mason, who until recently was the department’s executive director, cited two instances, in 1932 and 1957, where the head of the agency warned it lacked the funds it needed to run, according to October meeting minutes of the study committee. Services were cut and license fees raised in both instances, he said, according to the minutes. “If the Legislature does not act to increase the amount of General Funds appropriated to the department,” said the summary of his remarks, “the following biennial budget will require dramatic cuts in personnel and services, an increase in license fees, more General Funds, or a combination of all three.” Though the report didn’t make any specific funding change recommendations, it said the department should continue to assess whether additional general funds will be necessary to cover the costs of employee pay raises, retirement, and information and technology expenses. Sen. David Watters, a Dover Democrat who chaired the study committee, has filed a request for a bill that would require the general fund to cover the department’s “retirement contributions and technology services.” The report also recommended that Fish and Game bill other departments that use its services. Wipe away any moisture on surfaces - The most effective way to halt mould growth is by controlling the moisture levels in your home. Therefore, wiping away moisture on windows and walls will help keep these areas dry. Regularly airing out wardrobes and cupboards and using moisture absorbers in basements and wardrobes can also be beneficial. 3. Get a dehumidifier - This is a great option if your home has limited windows as it extracts moisture from the air and releases dry air back into the room. Warm up the room - While heating can be expensive, turning on the heat for a short period can help prevent mould by keeping rooms from cooling down. The ideal temperature for living rooms is between 20C and 23C, while for bedrooms and kitchens it's 16C. It's advised to avoid letting the temperature drop below 14C. 5. Check for and fix leaks - Look for leaks as dripping taps and pipes can promote mould growth, particularly in enclosed spaces like cabinets. Use washing up liquid - The same technique can be applied using washing up liquid and water instead of the spray. This method is also more cost-effective as washing up liquid can be bought for as little as 62p. Be careful when removing mould from wallpaper, as too much water can cause it to bubble. 3. Grab some white wine vinegar - This ingredient is highly effective in removing mould and can be purchased for as little as 35p. To remove mould, create a solution of one part vinegar and water in a spray bottle. Spray on the affected areas and leave for an hour. Afterwards, wipe away the vinegar residue with a damp cloth which should also remove the mould. 4. Purchase some bleach - Similar to white vinegar, a solution can be made using household bleach which can be obtained from 60p. Mix one part bleach and four parts water before using a cloth to wipe the solution across the wall, scrubbing gently to eradicate the mould.
